Family Support Case Manager for Regional Non-Profit
PathWays of the River Valley is in its 50th year of expanding opportunities for individuals with developmental disabilities.
As a Case Manager for Family Support Services, you will be a part of this legacy by providing service coordination and advocacy while linking individuals and families to existing community and state resources.
If you like assisting others in accessing necessary resources and broadening their involvement in the community, this may be the position for you.
Here is what you will do in this role:
Advocate and support individuals and families in a non-judgmental manner.
Increase opportunities for participation in the community by responding to the changing needs of the family.
Balance the use of informal and formal resources while maximizing family control over services.
Serve as a liaison with the community, state agencies, and service providers.
Understand, promote, and support individuals and families through planning the transition from the educational system to adult supports/services.
Facilitate the legal process for individuals and families as necessary including Guardianship, Power of Attorney, Advance Directive planning, and Representative Payee.
We are looking for:
Bachelor's degree preferred -- extensive case management experience may substitute for the degree A minimum of 2 years of case management experience A minimum of 2 years of human services experience Experience with developmental disabilities preferred Excellent verbal, interpersonal, and written communications skills At least intermediate proficiency in MSOffice Willingness/Ability to learn new software programs Willingness/ability to travel within the region/state as needed All PathWays' positions require the completion of a comprehensive background check and you must have reliable transportation and present a valid driver's license and proof of auto insurance.
You must pass a TB test (paid for by us).
